Pancreatic cancer remains a dismal disease with an incidence that is on the rise even in a low incidence region like India. In recent years, there have been significant advances in the understanding of pancreatic cancer largely due to strides in modem molecular biology, pathology and radiology. While molecular biology continues to provide an optimistic vision for future treatment approaches to this deadly disease, the recent identification of PanIN‘s and resection margins in pancreatic cancer have important implications for current day-to-day practice. Also, better understanding of IPMN and its management has improved the scope of pancreatic tumors. These rapid developments have ensured that pancreatic cancer has evolved from being a purely surgical problem into a multidisciplinary concept. However, surgery firmly remains the treatment mainstay and surgical concepts and techniques continue to get refined. This has resulted in ever improving peri-operative outcomes and procedures of greater complexity are being performed more frequently. Controversial aspects of surgery for pancreatic cancer are often not evidence based and there is a need to generate higher levels of evidence. This is possible only by collaborative efforts from centers of excellence. Also, centralization of pancreatic surgery has gained ground as an important concept to improve outcomes from such centers of excellence.
